Our Location We're located on the campus of William Beaumont Hospital in Royal Oak, Michigan. The hospital is at 3601 West 13 Mile Road, just West of Woodward Avenue. Our office is inside the Vascular Services Center, southeast of the main hospital building. The closest entrance is off Coolidge Highway. Turn left at the 3-way stop as you enter the hospital campus. You'll see South Hospital Drive on your right, followed by two smaller driveways. Enter at the second driveway. Patient parking spaces are on the left. We're in the single-story building with a canopy in front of the entrance. You may view and print a map of the campus by clicking here. Filling out this form at home is optional, but it will save you time at our office.   New Patient Form (Microsoft Word format) Initial Appointments Most patients can expect to have Vascular Ultrasound tests during their initial appointment. These tests are usually conducted on-site. Please Bring 1. A list of all the medications you are currently taking. Include the dosage and frequency found on the Rx label. Please list non-prescription medicines and supplements as well. 2. The name and phone number of your primary care doctor. 3. The name and phone number of the referring doctor (if different from your primary care physician) and any lab results or X-ray films provided by the referring doctor. 4. Health insurance card(s) or other information about your health coverage.
